- Green Card (even if expired)
- FOIA requests/documentation (if any)
- All passports used in the last 5 years
- Home/school/employment history for the last five years (or three years if married to a U.S. citizen)
- Children’s (under 18) information (date of birth, A#, current address)
- Marital history (Information about current and prior spouses including: dates of marriage, date of birth, and place of birth)
- If you have been arrested, cited or given a ticket, you must have your certificate of disposition and arrest record for each incident (if any)
NOTE
If you plan to apply for a fee waiver, please ensure that you bring your 2025 most recent tax returns, Food Stamp or Medicaid Award Letter (in English), or Supplemental Security Income (SSI) Letter (in English).
If you are not eligible for a fee waiver, a fee of $760 will be required. A credit card or bank account information will be used to complete a form for payment directly to USCIS.
